Before your fist visit…
Prior to your first appointment, you will need to complete new client paperwork. You can complete your paperwork ahead of time (at least 24 hours prior, if possible) and mail, fax or email the completed forms to us. You can save time by completing the NEW PATIENT FORMS. If you are working with other health providers outside of Schwieters Medical, we encourage you to complete a RELEASE OF INFORMATION to allow for easier care coordination between your providers. Note, a separate release is required for each provider.
Should you be arriving in-person for your appointment, please arrive 20-30 minutes in advance to allow time to complete the necessary paperwork and bring any insurance cards, as well as your government issued ID. If your insurance plan requires a copay, that fee is due at the time of service.
Additionally, if you have arranged with our office to pay privately for your visit, that fee is also due at the time of service.
